BACKGROUND: The positive benefits of immediate prosthesis breast reconstruction (IPBR) are incontrovertible. During the COVID-19 pandemic, health care resources became scarce. The implementation of outpatient immediate prosthesis breast reconstruction (OIPBR) can improve the efficiency of medical care and reduce viral exposure. Very few studies have focused on OIPBR and this study aimed to fill this gap by evaluating outcomes of OIPBR compared with traditional hospitalization IPBR (THIPBR) in terms of complications and quality of life. MATERIAL AND METHODS: The study enrolled patients undergoing IPBR at Tianjin Medical University Cancer Institute and Hospital between January 1, 2020, and September 30, 2021. Outcomes were defined as postoperative complications and quality of life before reconstruction and at 3-month follow-up. Quality of life was assessed by BREAST-Q questionnaire. Inverse probability of treatment weighting and propensity score matching (PSM) were applied to adjust for confounders. RESULTS: A total of 135 patients were enrolled, including 110 with THIPBR and 25 with OIPBR. After matching, baseline characteristics were well balanced. Patients with OIPBR had lower rates of lymphedema on the surgery side (p = 0.041) and readmission (p = 0.040) than patients with THIPBR. No statistically significant differences in the quality of life metrics of psychosocial well-being, sexual well-being, satisfaction with breast and physical well-being of the chest were found between the two groups. CONCLUSION: OIPBR is a safe and efficient alternative to THIBPR during the COVID-19 pandemic. It is recommended when medical conditions allow to conserve medical resources. Accelerated technical training for the performance of OIPBR at the hospital level should be expedited. Las estrategias para el éxito en la rehabilitación bucal requieren de la interrelación de varias disciplinas que en conjunto logren resultados predecibles y duraderos. La visión individualizada de cada área de especialidad puede conllevar a no ofrecer la mejor alternativa de tratamiento, es por ello que la valoración, el diagnóstico y la planificación del caso clínico debe ser realizada por un equipo interdisciplinario para evitar esta situación y crear una sinergia en donde el «todo sea mayor que la suma de sus partes¼. El objetivo de este trabajo es presentar un caso clínico en el cual intervinieron varias áreas de especialidad: periodoncia, prostodoncia, cirugía oral y patología bucal, logrando devolver la función y la estética a través del manejo interdisciplinario (AU)


The strategies for success in oral rehabilitation require the interrelation of several disciplines, which together, achieve predictable and lasting results. The individualized view of each specialty area may lead to not offering the best treatment alternative, which is why the assessment, diagnosis, and planning of the clinical case must be carried out by an interdisciplinary team to avoid this situation and create a synergy in where the «whole is greater than the sum of its parts¼. The objective of this work is to present a clinical case where several areas of specialty intervened: periodontics, prosthodontics, oral surgery, and oral pathology, thus achieving the return of function and aesthetics through interdisciplinary management (AU)

OBJECTIVE: To assess the clinical outcomes of implant-supported full-arch immediate prosthesis over 6 months of functions and analyze the risk factors of prosthetic complications. MATERIALS AND METHODS: This retrospective cohort study included patients who were treated with implant-supported full-arch restoration under immediate loading protocol between April 2008 and June 2016, and who wore the immediate prosthesis for more than 6 months. Medical charts were reviewed for the patients' general information, implant information, prosthetic information, and details of any prosthetic complications. Prosthetic complications were classified as follows: class I, prosthesis loosening; class II, fewer than three artificial teeth fractured; class III, three or more artificial teeth fractured; and class IV, resin base fractures. A cox proportional hazards ratio model was adopted to analyze the potential risk factors for class IV complications. RESULTS: A total of 114 patients (mean age, 56.73 ± 10.19 years) and 144 prostheses were included. The average wearing time for the immediate prosthesis was 21.9 months, and 62 (54.39%) patients experienced prosthetic complications, 30 of whom suffered from complications more than once. The most common complications were class II complications (12.3%) during the first 6 months of functioning and class IV complications (28.1%) during the entire function period. Class II complications were more common in the anterior region, while class IV complications occurred more often in the posterior region. The multivariable Cox proportional hazards regression model showed that a prosthesis in the maxilla (HR = 3.37, P = .001) and fiber reinforcement (HR = 0.39, P = .021) were significantly related to class IV complications. CONCLUSIONS: Implant-supported full-arch immediate prosthesis functioning over 6 months have a high prevalence of prosthetic complications. Fiber-reinforcement could reduce the prevalence of class IV complications when acrylic immediate prosthesis functioned longer than 6 months. Avoiding using the anterior teeth of the immediate prosthesis to tear pliable but strong food may prevent tooth fracture.

RESUMO

Se presentan los casos clínicos de 3 adultos con dientes extraídos, atendidos en la consulta de prótesis dental del Policlínico Docente 30 de Noviembre" de Santiago de Cuba, a quienes se les colocó prótesis inmediata como única solución ante el deterioro bucal que presentaban. Con el proceder realizado se logró la recuperación estética de los pacientes, así como la reinserción en las actividades sociales y laborales, evitando así las afectaciones psíquicas que suelen aparecer


The case reports of 3 adults with extracted teeth are presented. They were assisted in the dental prosthesis service of 30 de Noviembre Teaching Polyclinic in Santiago de Cuba to whom immediate prothesis were placed as the only solution due to the oral deterioration that presented. With this procedure the cosmetics of the patients was recovered and their reintegration to the social and labor activities was achieved, with which the psychic disorders that usually appear were avoided

RESUMO

Se describe el caso clínico de una paciente de 62 años de edad, quien temía a los anestésicos dentales convencionales, pero tenía gran preocupación por rehabilitar su estética con prótesis inmediata. Se decidió aplicar anestesia acupuntural y realizar las extracciones dentales para que recibiera el tratamiento deseado. No se manifestaron reacciones adversas y la paciente quedó satisfecha por los resultados obtenidos, así como por las ventajas que ofrece esta terapia natural.


The case report of a 62 years patient is described who was afraid of the conventional dental anesthetics, but she had great concern to rehabilitate her aesthetics with immediate prosthesis. It was decided to apply acupunctural anesthetic and to carry out the dental extractions so that she received the desired treatment. There were not adverse effects and the patient was satisfied with the results obtained, as well as with the advantages of this natural therapy.

RESUMO

En un esfuerzo por ayudar a estos pacientes y teniendo en cuenta las ventajas quirúrgicas, biológicas, estéticas, psíquicas y fonéticas que la rehabilitación con prótesis inmediata representa, hemos incrementado este tratamiento y de esta forma elevar la calidad de vida del adulto mayor. En este trabajo, presentamos un caso de una paciente de 76 años de edad socialmente activa, a quien se le realizó una prótesis inmediata en la Clínica de Prótesis del Policlínico 19 de abril. Se evolucionó clínicamente a las 24 horas para evaluar la cicatrización, dolor, inflamación y la satisfacción de la paciente después de recibir el tratamiento. Se concluyó que la prótesis inmediata actuó como vendaje, protegiendo el coágulo, favoreciendo su epitelización e impidiendo la aparición de infecciones secundarias, con menos dolor, y limitó la inflamación. La paciente sufrió un mínimo de desintegración de su personalidad física y psíquica; se manifestó muy satisfecha con nuestro trabajo.


In an effort to help geriatric patients and having in consideration the surgical, biological, aesthetical, psychological and phonetic advantages that immediate prosthesis rehabilitation provides, we have increased this treatment and by that, we provide a better life style to the elderly. In this work, we present a case of a female of 76 years , socially active, to whom we implemented the immediate prosthesis at The Prosthesis Clinic of the “19 de Abril” Policlinic . She was clinically followed after 24 hours to evaluate scarring, pain, swelling and patient's satisfaction with the treatment. It was concluded that immediate prosthesis acted as a bandage, protecting the clot, favoring epitelization and avoiding the appearance of secondary infections with less pain, limiting swelling. The patient had a minimum of damage to her self esteem, manifesting her agreement with the results of our work.
